- Intensive case management (2–3x/week)

- Re-entry and post-treatment stabilization
- Community support & resource navigation
- Psychiatric evaluation & medication management
WHO QUALIFIES:
- Medicaid recipients
- Homeless or unstably housed
- Severe mental health diagnosis
- Returning citizens
- Recently discharged from detox, hospitalization, IOP, or residential treatment
